  • ADOPT-200. This paper is an adoption petition, which will officially announce your intentions to adopt the child. It is filed with the county clerk in the county in which you live.
  • ADOPT-210. This paper announces your agreement to the adoption process. If the child you are intending to adopt is 12 years old or older, he or she will also have to agree to the arrangement. The judge will inform you when to sign this form.
  • ADOPT-215. The judge will sign this form if your adoption is completed.
